outreach support

A hand reaching out

Outreach Support provides real solutions to the problems faced by children and young people who are, or have been, in the care system. We recognise that children and young people often need help, support and guidance even if they aren’t in residential care.

Our extensive knowledge and experience of young people in the LAC system has enabled us to develop this highly flexible and individual-specific service. Designated outreach workers work with the young person and those around them to identify and solve the problems affecting them. The aim of this work is to prevent placement breakdown and build a stronger foundation so the placement has longevity. The Outreach Support service is available within the following settings:

Foster placements
Family placements
Mainstream or specialist education
Living independently

These situations are not exclusive, due to the diverse needs of young people who require support and guidance.

Outreach workers are, essentially, professional role models who work alongside a Case manager to develop and implement a care plan specific for the young person and their needs. A comprehensive reporting and assessment system is in place to record all findings and progress. This also includes feedback from the young person.

We utilise Landsker Education, where appropriate, to provide educational focus and activity. This ensures the service is wide ranging and addresses an important area of a young person’s life. The input can range from formal curriculum- based work to more flexible independent skills programmes to accessing work experience and vocational placements. These can also include therapeutic intervention work to address areas that are of concern, specifically Family Therapy work.
